To update your name, please complete the Inspection Notice or Claim Form you received and attach with it proof of name change. Examples of documentation you may provide is a copy of a marriage certificate, divorce decree, name change decree, and/or a driver's license.
Top - Why are former co-tenants listed on the Notice I received?
According to Defendant's records, all those listed on the Notice are or have been tenants at the address at which the Notice was mailed to. All names listed on the Notice must sign and complete the form to be considered valid.
Top - What if the co-tenants are no longer a resident?
Please complete and sign the Notice to the best of your ability. All Notices will be thoroughly reviewed by the Settlement Administrator. If additional information is required, they may contact you via mail.
Top - What do I do if I moved to another JD home location?
Please complete the form you received indicating you moved to another JD home. Please also provide your new JD home address, so the Settlement Administrator can update their records.

When completing your Notice, please indicate the correct dates of your tenancy so the Settlement Administrator can update their records.
